• Number of refills authorized
• Directions for use
• Authorization for generic equivalent 

Prescribing Veterinarian information needed:

• DVM Name
• State License # 
• Clinic Address
• Clinic Phone Number
• Clinic Fax Number

Authorized methods of Prescription delivery:

Fax the prescription from your office to our pharmacy at 800-979-2870

Call our pharmacy at 1-800-979-2869. Mon-Fri, 8am-5pm CST.  Your may also leave us a private message after hours at the same number. Or you may ask us to call you back directly if you wish.

Your Client may mail us the original Prescription to the above address.

We will keep all Authorized Client Prescriptions on file for future refills. 

Please note that A to Z Vet Supply offers only US FDA/EPA approved medications. We supply all prescription medications as presented by the manufacture.

Your patient and the Veterinarian information is kept in strict privacy within a secure facility.
